We're sorry, but Family Connects NJ nurse visits are not yet available in your county. But you can still connect to resources through your county-based Connecting NJ hub.
Connecting NJ is a network of county-based partners and agencies that act as a single point-of-entry system for a number of resources across state government to support birthing people and new and growing families, focused on streamlining referrals to obstetrical and prenatal care providers, access to support from community agencies, and assistance to help families thrive. It includes help for breastfeeding, infant developmental health, help finding a doula, access to FamilyCare health coverage, food assistance, public benefits like TANF and SSI, legal assistance, and more.
Connecting NJ provides information and referrals for both health and social resources. They can also screen families for eligibility for other home visiting programs available in your county. As part ofFamily Connects NJ, we have community alignment specialists stationed at each Connecting NJ hub to help develop resources for expecting families and families with newborns.
