At-home nurse visits for a healthy start to a new life.

Family Connects NJ provides free and voluntary visits from specially trained nurses for families with newborn babies. Designed to make this critical time less overwhelming, this new program helps us improve health outcomes for newborns and parents throughout New Jersey. It’s completely voluntary, and is free for any family, regardless of their income, insurance coverage, immigration status or family make-up.


How the program works:

First, families will sign up for the program here on this website. Then, a Family Connects NJ team member will contact them to schedule their appointment and answer any pre-visit questions they may have. Because there is a short window after birth for Family Connects NJ to visit each family at home, we encourage families to enroll prenatally.

Then, after delivery, a nurse visits the family’s home to check on the physical and emotional health of the parents and newborn, answer questions, connect the family to community resources, and ensure they have a medical home or primary care practitioner. In the event the family is mourning the loss of their baby, the nurse can provide specialized support and resources during the visit.


Evidence-based care for healthier outcomes.

The Family Connects NJ program uses an evidence-based, universal approach to supporting newborns and their families. The model has been studied in two rigorous randomized, controlled trials. Researchers found that by the time a baby was six months old, when compared with non-participating families, the participating families had:

  • Greater community connections - 15% more than non-participants
  • Improved mother mental health - mothers were 28% less likely to report postpartum clinical anxiety
  • Reduced need for emergency medical care - ER visits and overnight hospital stays reduced by 50% in the baby’s first year
  • Mothers also reported significantly more positive parenting behaviors, such as hugging, comforting and reading to their infants
