Schedule Your Visit -- County Select
Family Connects NJ is an exciting new initiative that will allow the parents / caregivers of a newborn child to receive an in-home nurse visit within the first two weeks after the child's birth. The trained nurse will be able to check-up on the health of mom and baby, screen for potential postnatal health complications, answer questions about welcoming a newborn, connect the family to resources within their community and address any unexpected changes the family may be experiencing.
This program is being rolled out in the state of New Jersey over a few years, with services initially available in Essex, Middlesex, Mercer, Cumberland and Gloucester counties beginning in January 2024. If Family Connects NJ nurse visits are not currently being offered in your county, you may still be able to connect to help through one of New Jersey's county-based Connecting NJ hubs.
