Family Connects NJ is a groundbreaking new program designed to transform maternal and infant health in New Jersey. Through Family Connects NJ, families can request a visit from a specially trained registered nurse within the first few weeks after a newborn’s birth. Family Connects NJ is part of the Nurture NJ Strategic Plan, which aims to reduce maternal mortality in New Jersey by 50%, eliminate disparities in birth outcomes, and make New Jersey the “safest and most equitable place in the nation to raise a baby.” In addition to checking on the physical and emotional health of the newborn and parents, a key component of the Family Connects model includes assessment of a family’s other needs, and connecting them to community resources.

Family needs are not limited to only those families with demographic risk factors – all families have needs around the birth of a child. When the Family Connects NJ program is fully implemented, this service will be available to all New Jersey families welcoming a newborn – at absolutely no cost to the family. The service is also open to bereaved families, resource parents, and adoptive parents. New Jersey is only the second state in the nation to legislate such a program statewide, and when we are done, we’ll be the largest such program in the country.
