Good news! Family Connects NJ nurses are visiting families in your home county. In order to schedule your visit, we just need a little more information about you and your family's experiences. Please select a statement that best applies to your situation below.

NOTE: When scheduling your appointment, remember that your Family Connects NJ home visit should ideally occur within 2 weeks after your newborn's birth!

• I am pregnant, but less than 36 weeks gestation.
• I am pregnant, and I am at least 36 weeks gestation.
• My newborn was born less than 30 days ago (for birth families, adoptive families, or resource families).
• My newborn was born more than 30 days ago, but less than 12 weeks ago (for birth families, adoptive families, or resource families).
• I experienced a stillbirth or neonatal loss less than 30 days ago.
• I experienced a stillbirth or neonatal loss more than 30 days ago, but less than 12 weeks ago.
• None of the above statements apply to me and my family at this time.
